Our Process

The Senior Care Guardian Process

Choosing assisted living is a significant life step that warrants open conversations with your family and loved one. Transparent communication helps reduce stress and ensures a smoother transition. At Senior Care Guardian, we offer our guidance-from consultation to move-in-at no cost to you, as we are supported by our partner communities.

01

The Initial Inquiry

When you connect with Senior Care Guardian, we’ll talk through your loved one’s
specific needs, medical and personal preferences, budget and desired location
to find the absolute perfect fit.

02

Personalization List

Based on the details you share, an advisor will find tailored senior living options
and recommend communities in your preferred location.

03

Streamline and Optimize

The advisor will assemble and carefully choose the appropriate communities on
the call and answer any questions or concerns based on expertise and
specialized knowledge of the communities.

04

Scheduling The Tour

Touring the community will be the most important first step in the process
allowing you to make an informed decision before placing your loved one.
The advisor will reach out to communities and schedule tours at your earliest
convenience.

05

Preparing For The Tour

The advisor will support you with helpful resources including a ‘touring
checklist” to assess facilities for safety, hygiene, and quality of care.

06

Touring

You will visit communities and meet staff members, evaluate living quarters and
accommodations for cleanliness, care, and safety.

07

Move-in Support

At Senior Care Guardian we are with you every step of the way throughout your
journey. After a community is selected, the advisor provides support for the
move and logistical planning.

Physician’s Report for Residential Care Facilities for the Elderly (RCFE)

Admission to any assisted living community requires that a potential resident submit an RCFE (also know as a Physician’s Report or 602) prior to moving in. This form is completed and signed by the potential resident’s primary doctor to assess current functional capacity, mental condition and any other pertinent health-related issues or considerations.
